Service Description
For the ADHD assessment it is important to get information on childhood and current symptoms from another person who knows the patient well (known as an informant). These forms should be completed by an informant before the assessment. The patient should also complete forms before the assessment about current ADHD symptoms and functioning and the Wender Utah Rating Scale score about childhood symptoms. Dr Huda will also ask you to complete a form giving details about your GP and your medical and family medical history. If you are unable to complete the forms then Dr Huda may able to complete the assessment if you have someone present during the consultation who knew you before the age of 12 and can also answer questions as to how you are currently. Please note Dr Huda will do a careful assessment and this means there is no guarantee that he will confirm a diagnosis of ADHD. If Dr Huda thinks a diagnosis of ADHD is likely to be present, then he will send you an information pack about ADHD. Currently Dr Huda does not offer treatment for ADHD. Even if Dr Huda confirms you are likely to have ADHD this does not mean your local NHS service will accept the diagnosis or fast-track you for treatment. Assessments typically last 60 minutes. Assesments will usually take place on Monday afternoons and evenings but there may be some flexibility for other days and evenings depending on Dr Huda's availability. Cancellation Policy
Dr. Huda offers assessments only, not treatment or prescriptions. The assessment fee is £600, non-refundable and payable upfront, which includes the correspondence to you and your GP. Cancellations require at least 48 hours' notice in order to have another appointment rearranged. |t is the patient's responsibility to make sure they have adeqaute equipment and internet access to make the online assessment possible
